Consent Forms for Patients Under 18
Before each eye exam the patient must sign the 20/20NOW consent forms. There are different legalities for patients under the age of 18 years old.

Parent or Legal Guardian Must Sign the Consents
20/20NOW Doctors can not see a patient under the age of 18 unless all consent forms are signed by the parent or legal guardian. The intake and consents can be sent out and signed by the parent remotely ahead of the eye exam. They can also sign the consents in office before the exam.
The 20/20NOW clinical team will hard stop an exam if the consents are signed by someone other than the parent or legal guardian. A non parent like Grandmom or big brother can only sign the consent if they have been granted legal guardianship over the child from the state and courts.
Attending the Eye Exam - Age 8-15
If the consent forms are filled out by the parent or legal guardian, someone besides the parent may accompany the minor child to the eye exam. Grandmom, an older sibling over the age of 18, or an Aunt can bring the minor to the eye exam.
Attending the Eye Exam - Age 16 - 18
If the consent forms are filled out by the parent or legal guardian, a minor the age of 16 or 17 can attend the eye exam alone (unaccompanied), or accompanied by a non-legal parent or guardian.
