Starting Intake Form

1. Log into the EMR on the tablet

2. Choose Start New Intake in the EMR

3. Fill in the patient’s basic demographic details and press Apply

4. If the patient exists in the system already, select their name from the list. 

Otherwise, skip to the next step

5. Choose the patient’s preferred language, select your store, and choose an 

exam type. If you selected a patient in step 3, press Continue with selected 

patient. Otherwise, press Register as a new patient.

6. On the next screen, choose In Person Intake

7. Hand the tablet to the patient and have them fill out the intake
