Price and Warranty for Eye Exams - BJ's Optical

Below are the prices to charge the patient for an eye exam and the follow-up timeline.  Payment for eye exams is due at the time of the exam, we can not invoice the patient later.   The contact lens exam includes a comprehensive eye exam.   We can not do just a contact lens exam, patient must have a full comprehensive exam by 20/20NOW as well. 

Pricing For Eye Exam

Eye Exam:  $74.99

Contact Lens Renewal Exam:  $99.99

Brand New Contact Lens Fit:  do not perform

Fee to fill out forms (DMV, etc): $25

Followup and Remake Policy

Eyeglass Check within 90 days of Original Eye Exam: no charge

Eyeglass Check after 90 days of Original Eye Exam:  $35

Contact Lens Follow-up within 90 days of original exam: no charge

Contact Lens Exam upgrade on a different day AFTER a comprehensive eye exam has occurred within the past 90 days:  +80 or +90 for New fit.
